Step 2:

Download, print or email if you aren’t able to complete the online application at this time. When finished, save your document and email the form back to us at premierhealthcareacademy@gmail.com

Step 3:

Once you complete your application, you will schedule a date and time to come into the school and complete application process.

Step 4:

Once all of the information is received and accepted by PHA staff, you will be granted access to the next training course available.
